Hospice care is provided wherever the patient lives.  A team of healthcare professionals and specially trained volunteers work to meet the emotional and spiritual needs of our patients and families as well as the physical needs of the patients.  A staff member is available to assist you 24 hours a day, 365 days a year.

Your Personal Physician:   continues to direct medical care utilizing the hospice staff as their eyes and ears.

Hospice Medical Directors:  consult with the patient’s physician and all members of the patient’s team to build a comprehensive medical plan.  (meet our hospice medical directors click here)

Nurses:  visit the patient as often as needed, managing the physical needs of the patient and providing care management.  Nurses also provide education to family members or caregivers about the disease process.  Our nurses are experts in pain and symptom control.

Hospice Aides:  provide personal care for the patient, including bathing, grooming, changing bed linens and other light household tasks.

Family Support Counselors/Social Workers:   offer support for the emotional, social and practical needs of the patient and their family.  They coordinate community resources to help families solve personal and financial problems created by a life-limiting illness.

Hospice Chaplains:  Upon request, hospice chaplains will help with the spiritual needs of patients and families.  Hospice serves people of all faiths, respecting individual beliefs. (for chaplains page click here)

Music Therapists:  treat pain, anxiety, depression, and disorientation through the use of live music.  (for music therapy page click here)

Volunteers:  are carefully screened and trained to provide support and companionship for patients and families, and can stay with a patient while a caregiver takes some time away. (for information on becoming a volunteer click here)

Grief Support Counselors:  offer assistance and support to family and friends before, during and after their hospice experience.  The Caring Tree program provides specialized grief support for children and teens. (for grief and loss pages click here)
