*Hospice Aide, Full Time, Nights

This position will work night shift for the Hospice House including weekends and holidays. Must have current Florida CNA License and a minimum of 3 years CNA experience preferably in Home Health or Hospice.

*RN BBH West – FT

RN needed for full time position.  This position will provide care for patients in Gadsden and Liberty County. May also provide RN support wherever needed within our 8 county service area.  Must have current Florida RN license. BSN preferred. *RN Care & Comfort – FT

*Transitions Caseworker

Assists clients and their families with issues that interfere with obtaining maximum benefits from medical care. Provides community resource coordination by performing the following duties. Essential job responsibilities include: Maintains effective communication with Hospice Admissions Department, Volunteer Department, and Bereavement with regard to referrals. Interviews client/family unit/significant others to assess patient's situation and potential psychosocial and discharge needs.

Develops and implements a discharge plan for the patient in conjunction with the patient/family significant others, and hospital social workers/discharge planners. Travel is required within the eight county area that Big Bend Hospice covers in order to meet clients and complete a bio-psychosocial. Bachelor’s degree in Social Work from a school accredited by the American Council on Social Work Education or related field. A Bachelor degree in a related field with additional years of social work experience may be substituted for BSW degree. Minimum of 2 years of experience in social work required.

*Transitions Manager

Responsible for the day-to-day management and marketing of the Transitions Program. Oversee quality selection, training and retention of volunteers to assist with the transition process. Assess appropriateness for admission to the Transitions program and offers supportive education regarding other choices if clinical care is needed – Hospice, Homecare, CCPC, etc.  Acts as a contact person with physicians, hospital discharge personnel, other outside agencies, clients, and families/caregivers to complete the Transitions Program admissions process ensuring clients/families have choices regarding providers from whom they qualify and elect services. Provides supportive education to clients/families/caregivers regarding care options. Master’s Degree in one of the helping professions preferred but experience in a related field will be considered; 3+ years relevant experience in a similar program and 2 years of program management experience required. Must have excellent communication skills and the ability to give oral presentations to diverse groups. Must be skilled in fostering positive interpersonal relationships and have a team oriented approach to all facets of leadership. Some travel required.

*RN Float – FT

*Housekeeper, Hospice House, FT

This position will work 40 hours per week on a rotating schedule that will include weekends. The position will provide daily cleaning of common areas, patient rooms as well as staff and public bathrooms. This includes but is not limited to: dusting, sweeping, wet mopping and vacuuming. A High School Diploma with a minimum of one year of experience cleaning in a health care facility is required.

*Dietician, PRN

Must have active Florida Registered Dietician License. This position requires a Bachelor’s degree in Food, Nutrition or Dietetics. Minimum of 1 year Registered Dietician experience. This position will work approximately 5-10 hours per month, as needed.    

*Counselor/Educator, FT

Responsible for implementing a comprehensive range of grief support activities and additional support services that inform, support, educate and guide individuals, families, schools, and organizations. Services take place in a hospice or community setting and include individual and group counseling, some case management, public speaking, networking and referral. Must have a Master’s Level degree in Social Work, Mental Health, Music or Art Therapy, Education or related field and prior experience working with children. This position may work nights and weekends and may occasionally work more than 8 hours per day.

*Medical Billing Specialist

Responsible for billing charges generated by the Agency’s third party sources and requires a professional individual with strong interpersonal and administrative skills.  Applicants must have at least two years medical billing experience with Medicare, Medicaid or private insurance filing.  Must also have experience in insurance verification, investigative billing and familiarity with EOB’s. Ideal applicant should have Associates Degree in business/related field or equivalent work experience. Medical Coding Certification preferred.

*Hospice Aide, BBH East, Part Time

This position will provide care to our patients throughout the Jefferson, Madison and Taylor county service area 3 days per week. Must have current Florida CNA License and a minimum of 3 years CNA experience preferably in Home Health or Hospice.
