Advance Directives

Planningtoday for the future.

Advance care planning at the end of life, is not Advance care planning.

Living will, advance directive, power of attorney… these terms can be confusing, but it is important that everyone understands them. Big Bend Hospice encourages all adults over the age of 18 to communicate their end-of-life care wishes to family and health care providers.

Big Bend Hospice uses the Five Wishes document and operates the PEACE (Planning Early About Care at the End) program, a Respecting Choices program designed to assist people in understanding, discussing, and communicating their advance directives through one-on-one conversation with a trained facilitator. Big Bend Hospice provides copies of FIVE WISHES, an advance care planning document published by Aging with Dignity, which focuses on addressing a patient’s healthcare wishes in the event that they cannot advocate for themself. This is an internationally recognized document, and in partnership with Aging with Dignity, Big Bend Hospice distributes them at no-charge to any person residing in the Big Bend region.

Request your copy of FIVE WISHES today! You may also call our office at 850-878-5310 to request a copy or schedule a PEACE session.
